Festival Permission Form

A permission form must be provided to the festival registrar for each child on the day of the festival. There are two ways to access the Central Florida Choristers Guild Permission Form for the upcoming Children's Choir Festival. Please select one of the following choices.

• Download the form to your own personal computer and then print out the Permission Form:
